For the issuance of a European Insurance Card, it is required:
- Insurance capacity check for the current year (EFKA – Insurance Capacity)ΕΦΚΑ – Ασφαλιστική Ικανότητα)
- The special European Health Insurance Card Application form completed in all fields
- Photocopy of ID or passport
- Individual Sickness Booklet
In case of receipt by a third party, authorization is required.

In the case of loss of the card, a Responsible Declaration is required from the beneficiary or, in the case of an indirect member, a declaration from the directly insured person with his/her original signature validated.
